Team bio: The eCLINICIAN MyChart Working Group is a multidisciplinary team from Alberta Health Services. The Edmonton Zone of Alberta Health Services implemented a patient portal that is tethered to a Zone-wide shared Electronic Medical Record. It allows patients to access to all of their laboratory, diagnostic imaging and many text reports, secure messaging with their health care team, and management of their appointments.

Team bio: Fraser Enabled is an interdisciplinary team from Fraser Health in British Columbia. The team implemented an e-visit solution to allow patients in one hospital to have a virtual consult with a specialist in another hospital.

Team bio: KLOOP is an App created by the McGill University Health Center. With this App, the KLOOP team implemented a two way communication channel between the patient and the care teams.

Team bio: MSH x Dash MD is a project of Markham Stouffville Hospital in Ontario. Partnering with a vendor, MSH x DASD MD provides emergency department patients with access to a smartphone application that provides aftercare information to patients, and also helps patients discover community care resources, manage medications and appointments, and provides an opportunity to submit feedback on their experience at the hospital.

Team bio: The Northern Health Authority covers over two-thirds of British Columbia’s geography, providing health services to predominately rural and remote communities. The Northern Health Authority has collaboratively produced SmartMom, a prenatal education program delivered through text-messaging, to offer a new accessible format to engage with pregnant women in British Columbia to improve health literacy, enhance health resource utilization and motivate positive health behaviour change.

Team bio: Tuberculosis Video Observed Therapy is a project of Alberta Health Services. The Calgary Tuberculosis Clinic, Clinical Telehealth Services and Unified Communications Services are piloting Microsoft’s Skype for Business. This application enables clients to join virtual appointments with their clinician to improve accessibility, reduce economic burden and maintain adherence to their tuberculosis treatment.
