According to the Central Community Care Access Centre (Central CCAC), 70 per cent of patients do not adhere to medication as prescribed after they are discharged from the hospital.
To address this problem, Central CCAC developed a standardized process for medication reconciliation. A pharmacist from Central CCAC will meet with the client right in their home, review their medications and identify any discrepancies and/or medication related problems. An electronic report is generated through their database, which is linked to the CCAC’s Client Health Record Information System, to be shared across the care team.
This solution has had a big impact, for patients and caregivers alike – ER visits due to medication problems have been reduced, and communication is improved across the continuum of care, and patients are getting better care.
