The Hamilton Niagara Haldimand Brant (HNHB) eHealth Office is responsible for making patients’ information available electronically to health service providers across the healthcare spectrum. Their solution is a web portal that provides physicians and clinicians with real-time access to their patients’ electronic medical information from regional hospitals, Community Care Access Centres (CCACs) and Oncology Centres. The tool is fundamental in bridging the gap between varying information systems spanning a wide geographical area and has helped to increase the speed of treatment, reduce repeat testing and improve communications with concise electronic information.
Innovation is extremely important to the HNHB eHealth team, and in 2005, their goal was to increase efficiency for health care providers, enhance work flow, and improve patient safety by having access to the right patient information at the right time. Since employing the portal, significant advancements have been made and it has spread from being used at Hamilton Health Sciences’ hospital sites to providing seamless, real-time integration of patient information from across South West Ontario.
