How does digital health support patients/caregivers to become more active participants in their health care?

It is simpler than using paper forms.

Share an experience where digital health had an impact, or could have had an impact, on you as a patient, caregiver or family member.

I was diagnosed with bilateral breast cancer about 10 years ago. My records were at a hospital other than where the surgery would be performed. I was asked to get my records from the other hospital, which I did and delivered them physically.

The theme of the 2016 Infoway Partnership Conference is “A Conversation About Digital Health”. How do you believe you can contribute to the conversation?

By sharing my experience and proving that digital forms would have saved time and effort for the medical staff and myself.

How will you apply and share what you learn from Digital Health Week and the 2016 Infoway Partnership Conference?

I am on a number of health-related boards/committees where I would be able to share what I learned at the Conference.
